Introduction to My Health
Online Care maintains a summary of your health information, which may include information from HMSA's records, as well as information from your online conversations with providers or information you self-report. In the My Health tab, you can view your health history and add more conditions, medications, allergies, immunizations, and procedures to your personal record.
From My Health, you can also print, save, or share a report of your health history with your personal care provider, online providers, or other care coordinators using the export options near the top of the page.
How to Complete Your Health History Information
You can use the Add buttons in each section to add a record to each area. During your online conversations, you will be able to share the information you enter here with your providers, and this information will also appear in your health summary. To get started, click Add in the appropriate section:
Any information you add here will be marked Self-Reported, and can be changed at any time. If you're an HMSA member, there may be information pre-populated in your health history based on prior claims; you will not be able to edit those records. If you believe there is an error in any of the imported information, please contact HMSA.
During your online conversations, we recommend that you share a summary of this health information with your providers to help them make informed decisions about your care.
Taking the Health History Questionnaire for Adults Assessment
One way to help populate your online health history is to take the Health History Questionnaire for Adults assessment. This assessment provides a general health overview, and your answers will be used to populate your health history with common medical information, such as overall wellness information.
