Introduction to My History

My History stores reports of your Online Care activities, including online conversations, assessment reports, self-care tool reports, and educational materials you have accessed. You can use My History to save these reports or share them with your primary care physician, online care providers, or care coordinators.

How to View Past Conversations

At the conclusion of each Online Care conversation, your provider will have an opportunity to add more notes, diagnoses, recommendations, or prescriptions to the conversation report. Once the report has been finalized, you will be able to access it in My History.

To find a report for a completed conversation, select Completed Provider Conversations from the View: drop-down list.

When browsing conversation reports, you will be able to see the complete chat transcript of the conversation, as well as any provider entries, such as diagnoses or prescriptions.

How to Save and Export Reports

It is important to use the export tools in My History to save and export reports of your online care for your records. Exporting reports can also help you keep the doctors you see in person up to date on care you receive online. Within the My History tab, you can use the export tools shown below to print, save or export reports:

  • Print: Print and archive your reports.
  • PDF: Save the report to your computer in PDF format.
  • Message: Send the report to another Online Care user via secure message.
  • Email: Send the report to a third party via email.
  • Copy to HealthVault: If you have enabled access to HealthVault, you can save the report to your HealthVault record.
